INTENSIVE IN COMMUNITY

Intensive In-Community Services (IIC) are targeted, goal-oriented clinical interventions designed to address the emotional and behavioral challenges of youth with moderate to high needs. Delivered by licensed behavioral health clinicians, these services can take place in the youth’s home or a suitable community setting. The primary goal is to stabilize and support the youth and their family, working collaboratively with the Child Family Team (CFT) or other identified support systems. IIC is a short-term, solution-focused approach that helps youth and their families or caregivers gain insight and improve functioning at home and in the community. Eligibility for IIC is available to youth receiving Care Management or Mobile Response and Stabilization Services (MRSS), as well as those transitioning from Out-of-Home (OOH) treatment back into the community, through the NJ Children’s System of Care.

BEHAVIORAL ASSISTANCE

Behavioral Assistance involves targeted, outcome-focused interventions that are part of a comprehensive, written care plan developed by a licensed clinical behavioral healthcare practitioner. These services are designed to address specific target behaviors, helping both the youth and their family develop and practice healthy, positive coping strategies. The interventions focus on improving emotional and behavioral regulation. Youth who benefit from Behavioral Assistance Services are expected to show lasting positive behavioral changes, leading to better daily functioning, an enhanced quality of life, and stronger skills across various areas of life.

RESPITE

Respite services are designed to offer supervision, allowing families caring for children, individuals with developmental disabilities, or other family members to rest and recharge while ensuring their loved one's needs are met by our trained team members. At Connections Are Real, we provide respite care with a unique therapeutic approach, either in the home or within the community, and these services are available throughout the State of New Jersey.

PARENT COACHING

Parent coaches, also known as parent educators, are trained professionals who assist parents in understanding and managing common parenting challenges, such as discipline and communication. They also offer valuable guidance during significant life events, including adoption, divorce, family trauma, and other transformative experiences.
